Employer Educational Assistance Authorization Form
This form is used for employer reimbursement.
Money for Textbooks
Students who wish to use financial aid funds to pay for their textbooks must apply online. This program is limited to St. Louis area campuses (Westport, Winghaven, Downtown and Webster Groves) and online students.
Change of Address Form
For students who have had a change in either name or address. Change in name must be accompanied with copies of legal documentation.
Transcript Request Form

This form must be faxed to Academic Advising at 314-968-7166 for St. Louis and online students only.
Add Slip
For students to add courses to their schedule. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.
Drop Slip
For students to drop courses from their schedule. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.
Withdrawal Petition
For students to withdraw from a course after the drop period has ended. This form must be delivered to the Academic Advising office in person. Faxed forms will not be accepted.
FERPA Release
For students to use to give permission to the University to discuss your records with other parties named.
Off-Campus Study Request
Students should use this form to request permission to take courses at a non-Webster institution for Webster credit
Summer Institute Application
Non-Degree seeking students who wish to take classes during the Summer semester should fill out this application. Webster University reserves the right to refuse registration if the student does not have the pre-requisite course work.
Request for Good Standing/Enrollment Verification
Students should use this form to request official documentation from the University.
Summer Institute Application
Non-Degree seeking students who wish to enroll in a Summer course must complete this form. Students wishing to enroll in coursework requiring pre-requisites must submit a copy of their transcripts
Undergraduate Application for Readmission
Undergraduate degree seeking students who previously attended Webster University but have been out for a calendar year or longer must complete this form and an advising appointment before they are able to register.
